腹腔镜联合经阴道子宫悬吊术治疗老年盆腔脏器脱垂82例

摘要

Objective To evaluate the surgical outcome of laparoscopic and vaginal cervical suspension for the treatment of uterine prolapse in old patients. Methods 82 old women with uterine prolapse who had undergone the laparoscopic and vaginal cervical suspension in Anhui Provincal Hospital between July 2007 and July 2010 were prospectively studied. Pelvic organ prolapse quantification ( POP - Q) scores and life quality questionnaire were undertaken to assess the outcome of surgery . Results 58 of 82 patients (70. 7% ) had properative prolapse stage 2, and 24 (29-2%) had properative prolapse stage 3. The operative time of surgery was (76±21) min. The estimated blood loss was (50 ±26) ml. The postoperative hospital stay was (5.6 ±1.5) days After operation, there were significant improvements in the stage of prolapse. Two patients had symptomatic prolapse recurrence during the mean follow-up of (36 ±9)months. Conclusion Laparoscopic and vaginal cervical suspension is a simple and good therapy method for the uterine prolapse in old patients.%目的 探讨腹腔镜联合经阴道腹膜外子宫悬吊术治疗老年性盆腔脏器脱垂的有效性和安全性.方法 对行腹腔镜联合经阴道子宫悬吊术82例患者的临床资料进行回顾性分析,POP-Q评估手术效果,生活质量问卷评估生活状况.结果 82例患者中58例(70.7％)子宫脱垂Ⅰ度,24例(29.2％)Ⅱ度.腹腔镜联合经阴道子官悬吊术手术时间(76 ±21) min,术中出血量(50±26)ml,手术并发症包括泌尿系损伤1例,术后住院日(5.6±1.5)d.平均随访(36±9)月,患者主观满意率100％,客观治愈率94.4％,复发2例.结论 腹腔镜联合经阴道子宫颈悬吊术是治疗老年子宫脱垂患者的微创,复发率少,疗效好的手术方式.
