我院Ⅰ类切口围手术期预防用抗菌药物情况调查分析

摘要

目的 了解我院围手术期预防用抗菌药物选择是否符合“关于抗菌药物临床应用管理关问题的通知”(以下简称“通知”)的要求,使用是否合理.方法 通过查阅相关病例,对2009年7月—12月和2011年同期我院Ⅰ类切口手术围手术期预防用抗菌药物的使用率、选用药物品种、给药时机、用药持续时间及用药后临床效果等方面进行对比.结果 执行“通知”以来,我院围手术期预防性使用抗菌药物情况渐趋合理,使用率由98.43％降至81.34％,药品选择规范率由17.17％提高至68.13％,给药时机正确率由10.37％上升至50.42％,24h内停药比率由7.24％提高至32.77％.结论 “通知”前后,我院Ⅰ类切口手术预防性抗菌药物应用逐渐趋于合理,但仍存在使用率过高、术后用药时间过长等问题.%Objective To explore the implementation status of hospital ‘Rule' of preoperative application of prophylactic antibiotics and effectiveness of the Rule in protecting the operation field from infection.Method By searching the relevant medical records between July to December in 2009 and 2011,preoperativeuse of prophylactic antibiotics in type Ⅰ incision operations were compared and analyzed for their;use,drug choose,administration timing,drug duration and effectiveness.Results After the implementation of the ‘Rule',antibiotics use was rational gradually.The antibiotics utilization ratio decreased dramatically from 98.43％ to 81.34％.The correct:rate of drug choice increased from 17.17％ to 68.13％.The percentage of antibiotics for prevention lasting 0.5～2h ranged from 10.37％ to 50.42％ and the percentage of drug duration less than 24h raised from 7.24％ to 32.77％.Conclusions After the ‘Rule' was into practic,preventive application of antibiotics in type Ⅰ incision operations of our hospital is becoming more and more reasonable.However,there are still many problems such as high-frequency drug treatment use after surgery,long time in using drugs.
