目的：探讨病历及时归档管理对诊断分类相关组（DRGs）的影响，并对其医疗质量进行评价。方法：随机抽取某院2012年1月至2012年12月以及2013年1月至2013年12月各4000份病历，统计管理实施前后的DRGs入组及付费情况并对病案首页进行质量分析，计算缺陷率。统计患者入院及离院方式，根据离院途径对不同入院方式进行加权打分。结果：实施病历及时归档管理后归档率从41.80%提高到83.20%，DRGs入组率从9.65%上升到44.60%，DRGs付费率从7.28%上升到38.20%，3组数据在前后比较差异均具有统计学意义（P＜0.05）。病历总缺陷率从15.20%下降到8.68%，前后比较差异具有统计学意义（χ2=9.978, P＜0.05）。结论：实施24小时及时归档管理对DRGs具有很大意义，可明显提高DRGs入组率和DRGs付费率，降低病历总缺陷率，提高医疗质量。%Objective: To investigate the effects of rapid medical archive management on diagnosis related groups (DRGs) and medical quality assessment. Methods: The medical records from January 2012 to December 2012 and from January 2013 to December 2013 were randomly selected. Each group had 4000 cases. The DRGs grouping ratios and payments were investigated. The front pages of medical records were analyzed and the defect ratios were calculated. The admission and departure form of patients were investigated. The weighted scores of different admission forms were calculated according to departure forms. Results: The archive rate increased from 41.80% to 83.20% after implementing rapid archive management. The DRGs grouping rate increased from 9.65% to 44.60% and the DRGs payment rate increased from 7.28% to 38.20%(P<0.05). The total medical record defect decreased from 15.20% to 8.68% (χ2 = 9.978,P<0.05). Conclusion: It has great benefits of implementing 24 hours rapid medical archive management. It can increase the DRGs grouping ratio and DRGs payment ratio obviously, reduce the total medical record defect ratio and improve the quality of medical treatment.
