运用HFMEA降低医院药品冷链管理断链风险

摘要

Objectives:To explore the application effect of medical failure mode and effect analysis (HFMEA) in reducing the risk of hospital drug cold chain management.Methods:According to the principle of tracing method,using HFMEA method to evaluate hospital cold chain drugs daily management,failure mode and potential risk factor analysis,risk index (PRN) calculated in advance,formulate and implement effective preventive measures.Results:The application of HFMEA model,drug adverse events occurred in the hospital management of cold chain were significantly decreased;the effects of the failure mode of PRN drug safety management of cold chain value from 369 points down to 45 points,each department of the cold chain scission point drug from 29 before the implementation of the fall to 4,after the implementation of various departments of the cold chain drugs the broken chain time down to 12.5 minutes after the implementation from before the implementation of the 190 minutes,with statistically significant difference between before and after the implementation (P ＜0.01).Conclusion:The application of HFMEA method can effectively reduce the broken chain risk and reduce the incidence of adverse events in cold chain drug safety management,and improve the risk awareness of hospital cold chain drug management.%目的:探索医疗失效模式与效应分析(HFMEA)在降低医院药品冷链管理中断链风险的应用效果.方法:根据追踪方法学原理,运用HFMEA方法评估医院冷链药品日常管理流程,进行失效模式及潜在风险因素分析,计算事先风险指数值,制定并实施有效防范措施.结果:应用HFMEA模式后,医院冷链药品管理不良事件发生情况均有明显下降;影响冷链药品安全管理的失效模式RPN值由369分降到45分,各部门冷链药品断链点从实施前29处下降至实施后4处,各部门冷链药品断链时间从实施前190分钟下降至实施后12.5分钟;实施前后比较差异具有统计学意义(P＜0.01).结论:HFMEA方法的应用可有效降低医院药品冷链断链风险,降低冷链药品安全管理中不良事件发生率,提高医院冷链药品管理风险意识.
