目的 探讨上颌垫式矫治器联合舌篓矫治乳牙期前牙反对牙面及语音功能的影响.方法 选择学龄前前牙反患者20名,平均年龄4.4岁.随机选择10名患者佩戴传统上颌垫式矫治器,另10名患者佩戴上颌垫式矫治器联合舌篓,比较两组治疗前后的头影测量值和语音变化.结果 治疗后两组间头颅侧位片测量值无显著性差异(P＞0.05).两种矫治器正畸治疗前后SNA角、NA-PA角有显著性变化(P<0.05),SNB角、ANB角矫治前后无统计学差异(P＞0.05).U1-SN角、U1-NA角、U1-NA距矫治前后有显著性差异(P<0.01).上颌垫式矫治器联合舌篓组治疗后语音清晰度明显升高.结论 乳牙期前牙反患者使用上颌垫式矫治器联合舌篓有利于上颌骨的发育,改变了上颌前牙的倾斜度,塞擦音清晰度明显升高,并且舌篓及舌肌功能训练有助于反的解除与语音的改善.%Objective To evaluate the clinical effects of maxillary bile plate combined with Longue baskel plaslic blocks appliance in the trealmenl of Class Ⅲ in primary denlilion. Methods Twenly palienls with anlerior crossbile in the primary denlilion were selected. The average age of the palienls was 4. 4 years. Ten palienls wore maxillary bile plale and the other 10 palienls wore maxillary bile plale combined with longue baskel plaslic blocks appliance. The cephalomelric analysis was performed before and after trealmenl,and the data were analyzed to compare the denloalveolar and speech changes of the two groups. Results There were no significant changes in cephalomelric analysis belween the two groups after trealmenl. ∠SNA and ∠NA-PA were different ( P < 0. 05 ), but ∠SNB and ∠ANB were not different belween the two groups before and after treatment (P > 0. 05). Z.U1-SN, ∠U1-NA and U1-NA distance were significantly different before and after treatment ( P < 0. 01 ) . The speech in patients treated with longue basket plaslic blocks appliance was more clear. Conclusion The maxillary bite plate combined with longue basket plaslic blocks appliance was more effective in improving the speech and beneficial to maxillary development.
