objective To investigate the efficacy and security of the silicone-rubber implantation in non-penetrating trabecular surgery(NPTS)through observed after operation.Methods (1)experimental study:The twenty healthy rabbits were divided into 2 groups by random methods.NPTS and the silicone-rubber implantation were operated in one eye of each rabbit,and the other eye was performed NPTS as the control to compare the two groups postoperatively,the follow-up period Was 12 weeks.The change of Intraocular pressures(IOP),anterior chamber,complication and so on were observed.Blebs were surveyed by ultrasound biomicroscopy(UBM),and each eye was examined by histopathology.(2)clinical study:NPTS combined with silicone-rubber and NPTS combined with SKGEL were operated respectively on 35 cases(47 eyes) of primary open angle glaucoma.The patients were followed up for 12 months.The visual acuity,IOP,filtration bleb and ocular reaction in the two groups were compared after operation.Results(1)experimental study:Compared with preoperative IOP postoperative IOP of the two groups reduced.There was significant statistical difference(P<0.05).Conjunctiva inflammation was observed in 18 eyes in experiment group.In control group,intraoperative perforation of trabeculo-Descemetg membrane was observed in 1 eye.Retention of reflective filtering bleb in experimental group was longer than that in control group by using UBM.There was significant statistical difference(P<0.05).Histopathologic change were investigated by immunostaining.The passages of humor outflow kept open in experimental group and became closed in control group at 12 week postoperatively.clinical study:Postoperative IOP after12 months weren't statistically difference between group A and group B(P>O.05).There was statistically difference between postoperative IOP and preoperative one in both group A and group B(P<0.05).Conclusion NPTS+silicone-rubber may play some roles in anti-fibrosis of filtering pathway after surgery and seems a sample,safe and effective procedure of anti-glaucoma operations.
