Abstract: The authors have developed a next generation PACS architecture. This PACS is based on DICOM, HL7 and other standards. The architecture is highly distributed and highly scalable, while allowing for high performance, significant workflow improvements and the minimization of network traffic. The possibility of interfacing to an existing HIS/RIS is allowed for in a fashion that addresses workflow issues, on a 'best effort' basis. THe integration of a commercial RIS into the PACS, so as to optimize workflow and minimize database proliferation, is allowed for. A cost- effective standard based distributed PACS can be built today. Such a system has numerous advantages over current non-distributed systems. These advantages include, improved network usage, improved workflow, true scalability without loss of investment and improved robustness, as there need be no single point of failure. !4
